This form is for SURGERY APPOINTMENTS ONLY, vaccinations are taken on a walk in basis only. This form was created to cut down on the number of calls coming into the clinic. IF YOU HAVE LEFT A MESSAGE ON THE PHONE AT THE CLINIC YOU DO NOT NEED TO COMPLETE THIS FORM as it just adds to the work load of our front office staff. IF YOU COMPLETE THIS FORM SOMEONE WILL CONTACT YOU WITHIN 24 BUSINESS HOURS (MONDAY – FRIDAY).

A reservation fee of $20 payable by VISA, MASTERCARD or money order is required to book your appointment.  This amount will be billed to your card and posted to your clinic account at the time you schedule your appointment.  It will be subtracted from your balance at the time services are provided.  If you must cancel or reschedule your appointment, please be sure to do so within 48 business hours before your appointment time (Monday – Friday) and your deposit will be returned to you.  These fees will not be waived.  Vaccinations, heartworm testing, feline leukemia testing, and other services are available at the time of surgery for additional fees. KCAAP accepts cash, VISA or MASTERCARD for payment of services.
